Tickets for the Rabbi's Retirement Gala are $200 each.

HOW MANY TICKETS WOULD YOU LIKE TO PURCHASE?

Please note that according to CRA rules, tax receipts can only be issued to the purchaser. If you are purchasing for others, only you will be entitled to the tax receipt. The value of the tax receipt for tickets is calculated after the event using CRA guidelines.

GUEST NAMES - Please list the names of every person you are purchasing a ticket for. One name per line. If you don't know, write NOT SURE YET and we will be in touch with you closer to the event.

TRIBUTE BOOK
We are creating a keepsake Tribute Book for every guest* that attends the gala.
Including a personal message in the Tribute Book is a special way to share your thoughts and feelings about the Rabbi and her impact on you/your family.

►  We will prepare the creative for you. Full and half pages can include a photo. Corporate logos are welcome.
►  The cost of your page is considered a donation and is entitled to a full tax receipt unless you include a corporate reference or logo, in which case it becomes a corporate tax write-off.
►  IMPORTANT: if you are purchasing a message with a group of people, the tax receipt will be issued to the person who makes the payment. If you would like the payment attributed to multiple people, please do not complete this section. Email gala@cityshul.com and we will arrange the payments for you.
